Consultation $50 Basic Assessment $150 Full Diagnostic Assessment $250 Psychotherapy/Counseling $120 per 50-minute session **Additional services are available based upon your personal needs.

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. We accept many major insurance carriers and plans, Call us if you have questions about your insurance coverage. You can also call your insurance carrier to verify coverage policies. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis. Our agency also participates in the Indiana Recovery Works Grant. Please contact our office to see if you meet eligibility criteria for a grant to assist you with payments for treatment and supportive services..

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $25.00 fee for the missed session.
